I am pretty sure that most nuclear armed countries have a deadman switch that would be effective -- a doomsday device. So, if one goes we all go.
North America, Eurasia, China, North Africa and the Middle East would no longer be habitable for a few generations -- even if you could survive. Nuclear fallout would reach other parts of the planet. Sunlight would be altered in spectrum. Nuclear warfare will not be a contained Chernobyl-like event. Every major city in a nuclear armed country is a target of some adversary nuclear armed county.
In case of nuclear attack: Seek shelter, put your head between your legs and kiss your sweet ass good-bye. Only the elite will be in survivable locations never able to walk the surface again in their lifetimes. That is a life sentence underground in a bunker -- nuclear prison.
Most of the survivors will starve to death while watching their flesh rot on their bones.
