I am pretty sure he is not schizophrenic. Every scene that happens in the movie has a meaning. The spiders have nothing to do with schizophrenia. They are representations of the women in his life and how he envisions them. You can never tell when there is something real going on or if what is going on is taking place in his mind. Every time he is having interactions with himself, this is taking place in his mind. The two guys are the same guy. But he is struggling with wanting a more exciting life. The school teachers girlfriend is never a spider but after the fight in the car and the wreck, the window is cracked and is shown as a spider web. When his mind was pursuing the idea of, and the excitement of infidelity, it still eventually lead to spiders. He sees the hooker with the spider head, I think he is starting to have a hard time distinguishing his sexual conquests from something that's going to eventually hold him down.
One could probably write about all of this for hours, because every single scene had an unexplained meaning. The story is shown to us like one of those 3d images where you have to cross your eyes to see whats really there. If nobody tells you to cross your eyes, so to speak, you pretty much can't see whats really taking place.

Sounds complex. It was a good movie but it put me to sleep two nights in a row trying to watch it
