In the UK on top of tax we pay National Insurance which is based on your income. 20 years ago i paid around £1200 a month NI. Thats around $1600 this went towards medical and pensions.
Even people who did not work or are on disability still had their health care covered by people who did work. So at least in the UK no medical treatment is free, someone pays for it!

Wow 20 years ago paying the equivalent of $1600. Today that would be over $2000, maybe even 2200.
Was that for just you or at least a family?
My personal insurance is $450 a month and it's pretty good. Just me, no family or dependent.
