In my state you must prove you are a citizen to register. Also, in my state, to get a drivers license you must show a birth certificate (or other accepted documents like military papers) proving you are a citizen. My mom, in her mid-70's no longer drives but had to renew her state ID. They made her track down her birth certificate in order to give her the ID card even though she has had an ID in this state since 1971.
You prove you are a citizen when you register then a few weeks before the election you ballot shows up in the mail. No citizenship, no registration, no voting. It's simple.
In the past several years there have been a total of fewer than 20 ballots that have been questioned for being possible fraud votes and in almost all of those cases it turned out the database just wasn't up to date and everything was fine.
