Incest porn is illegal (can't process for it with Visa/MC), but the market for it was always very big. The niche that became so popular over the last 5 years or so is the step sister/brother/mother niche. So it's not incest, as they are not real family.
With the divorce rate in western countries these days I assume many people grow up with a step sister/brother/mom so to many people this is probably a real fantasy they have or have had at one point in their life, so it probably relates to a lot of people.
