There is a yearly fee - 950 EUR, extension for next year is 800 EUR.
Debit cards are issued by one of the top russian banks. For example: tinkoff.ru/eng/, vtb.com/o-banke, sberbank.ru, gazprombank and a few others.
"Fee for loading the card from your bank account online" - depends on particular bank and what option is being used: wire transfer of card2card transfer. We do not charge anything here.
