Exploding Head Syndrome isn't necessarily painful. I only found out about it because I turned to Google after it happened to me a couple of times. I was on the verge of falling asleep when I heard a loud crash. Loud enough to get me out of bed to investigate but found nothing.
As the wiki page says it's not yet fully understood, but I think there's a good chance it's related to how the body dissipates tension as you fall asleep - it's supposed to happen gradually but every once in awhile you get that whole body jerk. Like that, but with your auditory system, or maybe just a transient stimulation of the auditory nerves.
