My theory boils down to greed. You see excellent creatives in small operations where the owner is also the creative talent and happens to have a talent for the work. But anybody that's getting adult creative work done by employees or (more often) contractors always ends up cheaping out. Sure, they can hire a professional and get top-notch creative work, but it will be expensive. The siren call of some English-as-a-second-language person willing to do the work for a fifth of the price always ends up winning.

Making this worse is that anybody who can write top-quality sales material is more likely to be doing it for their own account than for a wage or as piecework. Nobody wants to pay what that kind of work is worth, because they imagine the value is in their content rather than in the sales material. They won't pay fair value for sales creatives because it's a hefty fraction of the generated income, which they hope to mostly keep.
This is extremely spot on.

Adult program owners cheap out, and many wouldn't even know what a good creative was if it shit on their shoes.
