We were faced with the same problem...
Too far out of the city for DSL, too far off the main road for cable. We checked around and got a good deal on a partial T-1. It is supposed to be 768 up and down, but we get over 1mbps down, and about 500k up. It's more expensive than DSL/Cable/Satellite, but it's more than worth it to us. Our cost is $300 a month, and $700 up front for the T-1 router.
Nowadays I see full T-1's advertised for about $4-500 a month... we're thinking of changing.
