I'd say my favorite Italian restaurant at the moment is Da Emma in the old city. Casa Napoli in little Italy is good too, but you've probably already been there.

There's another restaurant in little Italy that I liked but I can't recall its name. Its about a block up St. Laurent from Casa Napoli and half the seating area is an indoor grape arbor. Its a bit on the casual side though (atleast at lunch).

There's also the little place about 2 blocks east of St. Laurent in little Italy where the mafia used to hold court. I wish I could recall the street and/or name of the place.

If you want to try something other than Italian, I'd suggest the Green Lime near the corner of Ontario and Pie IX. That's not exactly the neighborhood where I'd expect to find great food, but its definitely worth the trip if you like innovative cuisine.
