if it's a tgp or free site you gotta get tough on the hosting company, meaning a well crafted C & D letter to the hosting company putting them on notice that their ass is on the line as well as the infringer. Once a hosting company has been served notice that a customer of theirs is infringing - subsequent findings of more infringments by the same perpetrator can have significant legal ramifications for the hosting company.
If it's a paysite, pay more attention the 3rd party billing company. They have liability to worry about as well. Serious liability issues if they continue to process for a thief.
