Me with my limited knowledge of html

I need to add a couple of form fields to a webpage - what's the easiest way to do this? I'm pretty handy with Dreamweaver - Do I need a special script for this?

I need the surfer to be able to fill out the form - hit the submit button and the form get's emailed to me.
