it's just like the "smurf" DoS back in the day. everyone who was "on top of" the issue instantly patched their stuff, and it slowly trickled. but there's a lot of people out there who don't have regular contact with the internet, and don't keep up with things. also with companies constantly cycling through employees, old network admins who set up the equipment were gone, and the new ones didn't know how to fix it or no one ever wound up "owning" the equipment in question...

i guess that is slightly related to why there's a lot of people out there with directory indexes and hotlinking enabled.. either they don't know how or are completely oblivious to the fact that people can and will do that to them. in time they'll learn, once they get fat server fees for a while.
