Since most grenade will explode as they hit the ground and are made so that the blast force is directed in a V shape, it is a good idea to do the following if you are about to be faced with the explosion of a grenade; keep in mind that you have a couple of seconds to do the following:
1-Identify the location of the grenade fall
2-Dive as far as possible from the impact point
3-Lay down as flat as possible with your stomach on the ground and your leg stretched and your heels pointing toward the grenade
4-Cover your ears with your hands and open your mouth, with your face on the ground of course
5-After the grenade has exploded, get up, make sure the mofo doesn't have another one to launch and get the fuck outta there
This is not a 100% secure method, but it will greatly reduce the potential injuries that could come to you if you were to be faced with such an event.
