2257 does NOT require that a producer get any other information than the models' name, AKAs and a photo ID.
It does NOT require any other contact information be provided. No telephone numbers and no addresses.
If any affiliate asks you for this information they are more than likely trying to scam it out of you or they are totally ignorant and can't read the 2257 regs.
Just DON'T do it!
