What do you mean by 'approved'?
'Approved' invoice forms should be supplied by the party receiving you invoice. I suggest that you do a search for an invoice form in .ods or .xls and convert it into a .pdf -- lots of free ones out there. |

They are a bit slow, but they've always paid us in the past. Not sure if Paxum is still supported though.
I don't think the invoice look matters. There are also very quick online invoice generators that spit out PDF. Just Google "invoice generator". You can check in the CzechCash TOS what needs to be present, but it's pretty much the usual stuff. |

This is because of the employment laws ; an independent contractor is under contract and paid for the invoiced amount, as opposed to an employee paid monthly a salary or periodically either the same amount or a varying amount including a commission of some sort.
