Should there be something about being sober? Also it is a good idea to film the models agreeing to participate and state the date etc. At the end of the shoot you can again ask them the date and if you made them do anything they didn't want to do etc.

Not the kind of thing you want to use a sample for. Hire a lawyer to author it.
