I never thought it's even possible until I found the information about Operation Begemot when 16 ballistic RSM-54 rockets (can be equipped with 4 nuclear warheads each) were launched one-by-one from a single submarine of "Dolphin" class (APL-667 according to Russian classification) in 1991 right before the end of the USSR. So a single salvo sends 64 nukes within about only 1.5 minutes.
