I know it's being posted, but I had trouble getting a response from them but they just sent me this email. Maybe it will be a peace of mind for some. It is for me, as I've been very anxious getting my funds.

They asked me to send them their form... and forgot to attach the form! Duh! $3 ATM fee - very nice. That's good news.

From what I was told is that you only need the form for amounts larger then 10k
