Your suppose to include a link/url/somesort of "proof" that you actually have the original content. i.e a link back to a product page on a site you control.

otherwise the DMCA is to broad of a request if it just says "found on www.blahblahblah.com"

Did you see Item #3 on the instructions? Or even the Email Template in the video? Just curious.
This looks specialized or originally written for Envato Marketplace but might still be somewhat useful to anyone else, since the Email Template looks customizable.
