It's going to make a very small difference in rankings even assuming you've got everything else right. If you change the URLs on an existing site, you'll very likely go backwards for a few weeks even if you redirect from each old to new URL - months if you don't.

Having said that if I were starting from scratch I'd go for keywords in the URL. These are always likely to improve your clickthrough rates though aren't they?

I think the anchor tag is truncated at the ?.
Query strings are given little SEO weight that is part of mod_rewrite's SEO purpose

to include the sexy-babe-shows-all in a normalized URL. However, I remember years ago Yahoo's bot 'slurp' checked for the directory sexy-babe-shows-all also. Trivial today unless someone has noticed Googlebot behaving similarly ...

if the site has already pages indexed in google., then do yourself a favour and dont change the structure., it will take ages to recover from its position.

if its new site then you can change to
http://adultdumpsite.com/out.php?id=sexy-babe-shows-all

but in the long run it doesnt matter at all.,
e.g.: youtube , pornhub , redtube ,
