Form/Questionnaire script?
 

Anyone know of any EASY to use and FREE fill in form scripts w/ mailto when done? Not for WP since I found a few good ones for that..

There was a nice form script @ http://www.tomsnewbiebooster.com but just went there and the domain is parked. Maybe you can download it via http://www.archive.org

It was located on the "site map" section and the script is called "porn mail".

You want the free version of FormMail from TecTite. Use the Easy Wizard Setup to configure it for your site, upload it and use whatever form you create.
