Can you finish my form?
 

I need to collect data entered by users. I've created a form but would like it to look a little cleaner and it needs to be setup on the backend so that I can receive an email with all data entered by the users including an user uploaded image file. (**sorry to repost this as I think I put in wrong section first time around)
