The country you are doing business in makes a big difference.

A document with a list of stolen creditcards so you can make sales.

Besides the model release that contains the models name, birth date and signature, etc, you also need the signature (name, info) of the photographer and a witness.
Besides that, you need copies of 2 valid ID's and preferrably a picture of the model holding those 2 ID's.
