What dating sponsors do this:
 

Gives me some code so I can put a simple, customizable, plain form on my site which:

1. 'signs up' the surfer to the dating site and bombards them with whatever shit it takes to get them to become a paying member

2. Gives me that form info

3. Allows me to set my own successful submission page

Anything out there that does this?

You do that with your own join form and then send the info out to dating sites via API. There are services that can help with that like Lead Wrench and maybe some others.

None of the dating sites that I know of are going to just set this up for you. That information is valuable.
