Model Application Website Template?
 

I'm looking for a Wordpress template I can use to make a model application form for new talent. I need something simple that's plugin and play that would allow model applicants to submit 5 images and basic description and age. Any suggestions on a web site template I can use to accomplish this quickly?

Personally I use Quform on most my WP sites. Way better than Contact7 IMO. Only $25 on Code Canyon.

Anytime you allow uploads, you need to makesure the system will allow a large enough file.
