Question for those who act as their own custodian of records
 
So I want to be my own Custodian of Records and am filling out this form here to apply with the state of California:

Here it's asking for an ORI # and a mail code.

1) What the hell is a mail code? Zip Code is already on the form yet they want a mail code too. Searching it on Google yields no results.

2) What's an agency ORI #? Anyone know how to get one?

You fill out that form if you're working within the Department of Justice and are applying for approval to be a departments custodian of records (COR). Every department is required to have an assigned COR. Every department has an ORI assigned by the DOJ ( i.e. CA0301100 = Laguna Beach Police Department ORI )

The mail code is a 5 digit code assigned by the DOJ for your agency, it's not a zip code.

If you aren't within the DOJ and are applying to be an agencies COR as a private entity filling out this form would likely result in the agency requesting a BCIA 8016 to get you fingerprinted and in their system ( via LiveScan ), then determining if your background is suitable to someone authorized to handle the security, storage, dissemination, and destruction of criminal records furnished to the agency and who serves as the primary contact for the Department of Justice for any related issues involving those records and your department.

I have never done this one, but normally the ORI has to do with the Police or FBI (fingerprints etc) , I looked at the form, but not certain if the Agency, is meaning if your are required to submit fingerprints etc. just call the state office and ask. (but since its a criminal office form, I am thinking it has to do with the FingerPrints etc.)
