It's entirely possible that the lug nuts were torqued tight enough to specifications....but the bolts actually broke off with the lugnuts still on them...so the whole wheel/tire came off.

I'm thinking they probably sheared when the car went sideways? the lugs were tightened, if you think they weren't, you have little understanding of the pit process.... |
