Who to DMCA in this case?
 
A friend who is cam girl asked me to help her send a DMCA regarding this sites

So we e-mailed cloudflare and send a DMCA claim. No answer and it has been a week. Videos are still up.

The DMCA form on the site does not work.

What should be done in this case to get the girl's videos and photos down?
What can she do next?

That site runs on autopilot and hosted in eastern europe.
You can forget about dmca-ing it.

and if it is on autopilot where does it take the vids from? the csm sites themselves?

Firstly try to "talk" about it with upstore (the site where the content is uploaded) and get it removed.
DCMA protection cammodelprotection.com I have heard is getting results.

DMCA is for usa based hosting. Europe and other part of the world wont look at dmca

The most of them respect copyright and remove content on request.

As of cloudflare, try this form https://www.cloudflare.com/abuse
They'll reply, notify the abuser about the claim and give you the e-mail of his hosting provider.

If it's webcam shows you may be out of luck.

We are finding more and more of those are protected by the SCA (stored communications act)

The content is user submitted meaning she submitted herself.

We do DMCA's but we aren't cheap. If you keep having issues let me know.
