How do you come up with your website's user interface and layout?

I know there are plenty of articles about that on the web, but when you create pretty simple website, how do you come up with everything and do not forget to add something etc?

For example you want to create pornstar directory website. Do you simply checkout other directories and take down a list of navigation items/pages etc you want to include?
And then simply layout those in a sketch?

What is your method?

I just find one that I like the look of, download it and change the text - simples...

I am heavily influenced by the new simplified designs.
Most of these advances are in mainstream and social media websites.
In adult you can't get away from images and thumbs so you have to incorporate that aspect.

pushstate() and DOM content replacement in dynamically generated websites is what I am working on in the near future.

Also simplified navigation; If a new visitor cannot figure how to navigate around your website in first 5 seconds (effectively) -- then you have failed miserably in that visitor's mind. How likely is he to be motivated to buy what you are selling: being frustrated, confused or aggravated ?

Most adult websites lack focus and become a carnival of confusion IMO.
Also I am going to see if the JSON that schema.org may be helpful -- we will see I guess that was rather simple to code dynamically -- if it works ...

Less is more in the front end v. More is better in the backend -- the source code produced (on page SEO)

I am also a fan of simple front end. Nothing new, but my usual layout:
header with site name and tagline, top menu (about, contact etc), sidebar with categories or whatever and body.

I say ...keep it simple. Not too simple, but fairly simple and navigational friendly.
