i want one. can any server providier set this up for you or you have to buy something like a shell?

You must rent a shell with BNC and the provider should have a lot of vhosts. Each vhost is a different address for you.

Nah, I just bounce thru a proxy if I want to spoof my IP. Otherwise, I don't bother.

if you have your own server and the host allows u to, you can run your own bnc and have your host setup custom PTR on the IP, you need to have ur own domain tho
