Do you need the model to fill out a model release before every shoot or only the first?

Simple answer. A release and 2257 are two seperate items.

2257 = Government mandated documentation showing proof of age, done in an accepted format stipulated by the statutes. (nut shell)

Model Release = Contract between the photographer and the model, that ussually states who can do what with said material, who owns said rights and on which mediums, when the shoot took place, legal recourses, and hopefuly arbitration and choice of venue clauses. (nut shell)

By the way, yes a new release for every shoot is advisable, and get new or updated 2257 while your at it.
