You're brushing too hard, when you push on the brush that hard the bristles spread and aren't effective in cleaning. Gently go over your teeth and let the brush do the work. Best to buy a new one long before it gets that worn as its essentially useless at this point.

I change my brush every 5 weeks. I have a sonicare and it doesn't work well if the brush is all fucked up.

That's bad bad on your gums and enamel. Just the tips of the bristles should be doing the brushing, not the sides. Replace often and use a soft bristle.

I learned the hard way, I was wearing down my enamel and receding my my gums by brushing too hard.
