Does anyone know where to get a models release form that will work for content I want to do some of my own content. Thanks all in advance

Just google it there are tons of them out there for free find one that fits you best ;)

Two good suggestions in this thread. Going with an attorney is always the best route though.

if your aren't afraid of the FBI or local law, just google and modify one to fit your needs. If you are serious and understand what "having you shit pushed in" means, spend the 500-1000 for an attorney consultation. :)
