Buying content from the model

Can someone give me some advice as to how to buy content straight from the model? I have a fetish model wanting to sell me her photos but I don't know what forms I need. I am perfectly aware of 2257 and record keeping but I have never bought content from an individual, only content providers. Anyone know what forms I need? Also, what is a fair price for a pack of 30 or pack of 50. She will take these shots at home with her husband taking the shots and with a webcam.

1. signed model release that YOU give her that has your signature (mail it to her, have her sign it and mail it back -- you may even get her to take a picture of herself hold it up along with item #2)

2. scan/photo of government issue photo ID

Yes, that part is established now. I knew I needed those two items prior.

Where it says "Photographers signature" I can't sign that because I am not the photographer. Also it asks for a witness. Do I make her hubby sign as the photographer and then make them get a witness?

yes hubby signs.. you dont really need a witness. you might need an agreement with the photographer where "he" signs over the rights to you.
good luck with your approach.
