Best way to create a Wordpress 404 page?

What is the easiest and productive way to create a 404 page for a Wordpress blog? What you guys do with the 404s?

I add this to htaccess:

ErrorDocument 404 httx://www.mydomain.com

and all 404 go to mainpage
