CPM media with frequency cap specifically these guys are about the only ones who have this that I can think of. I use them on occasion myself and traffic converts pretty good if you know what you're doing. They also have pixel tracking so you can track whatever media you buy and see how it converts.

Form what I see they have 68mil US impressions daily and 26mil UK impressions on 300x250 banners and 1.7mil US popunders

i forgot, BlackLabelAds have CPM banners with frequency caps and pixel tracking - I've done so so with them when I ran campaigns - definately have to watch closely and weed out the bad apples if you're after conversions though

TrafficHolder has something like frequency cap - when you place an order you can choose it's either 1/day or 30/day uniques but this is skimmed/blind traffic, not banners

Choker has pretty good traffic but it's also not banners, not sure if he can do anything besides 30/day uniques - I know you can request traffic only from specific countries and he sells popunders
