Apparently the only design software you've used is MSPaint. I design quite a bit and and always group and name layers. When you have 100+ layers it's easier and minimizes file size. I'm not a birther, I just really dislike Obama.

Scan something into say a TIF or JPG image. Then convert that into a PDF using Adobe Acrobat. Open the resulting PDF in Illustrator, and there's layers all over the place.
