As it turns out, the beer elimination actually brought on the heart attack. His arteries were clogged, but drinking beer every day thinned his blood so it could still pass through the blockages. Once he stopped the beer, his blood thickened, and then boom, massive heart attack.

Duncan was in great shape. If anything he lived longer than he would have if he weren't a fitness buff.
Also...seeing that he suddenly decided to become a vegan 3 years ago kinda makes me think he was taking a doctors advice because they found something wrong with his heart.

Actors have to take full physicals before every role for insurance. So there is a good probability that they discovered a problem with his heart a few years ago, which prompted him to cut out all artery clogging foods and become a vegan.

Probably bought him a couple more years of life.
