The problem with NeatImage is it applies the filter to everything including things around the model. With Portraiture you can apply the filter only to the model's skin and nothing else, not even her clothes. The end result is the same, one just gives you much more control than the other.

Mutt, they're done by hand and using plugins. You can do it without plugins, since all plugins use things that exist in PS, but it would be too time consuming. For something fast and no plugins (I'm lying, I used noise reduction plugin first

this last one use no plugins at all

the idea was to go as far as possible without painting and spending less than 5 mins per image, and as you can see, unless the initial image has a certain quality (in general, they have, your pics quality is beyond bad) it's impossible to make them without plugins in a reasonable time, you really need to combine plugins and then do hand adjustments (painting, airbrushing, whatever) to get that kind of pics
