Sponsors that pay per banner impression?
Is money tree the only one? 1 cent per thousand banner uniques? That seems extraordinarily low.
Are there any programs out there that pay much better than this?

They are the only ones I know of. At 1cent/1000 you're probably better off using a 'pay per click' sponsor. For example, we pay $3/1000 clicks, (see www.fpctraffic.com), so all you need is a clickthru rate of 1:300 to do better than moneytree.
