LIST: Sponsors that require a W9

The other day I logged into an affiliate area to check something and noticed a bulletin about said sponsor requiring a W9, they posted the bulletin earlier this past spring. I checked my check logs, and sure enough, no payment for quite some time. Somehow I missed the original email they sent out, and I'm sure others have too. Since the W9 requirement is somewhat uncommon, I think it would be good to create a list of sponsors that do require them so we can scan it over and spot a sponsor we may have missed. You could have money sitting in an account you totally forgot about!

Please copy this list and add any additional sponsors you know of.

and forgot all year about it till last week. I hope i aint gonna have problems collecting it even thu its my fault.

I don't recall seeing requests from either Top Bucks or Lightspeed Cash. Guess I better check.

From what I recall, Lightspeed Cash sent me one in the mail after I started sending sales.
