Any upload script coupled with FFMPEG to do conversions.
http://ffmpeg.mplayerhq.hu/ Be aware people can upload bogus vids that when converted overflow FFMPEG and allow your server to be hacked. This exploit has been patched now I believe, however there will be new ones. There is an opening for hacks soon as you allow users to upload. -Ben

If it's for inhouse stuff I use php uploader...
For promos and actual sites, tevs or tube cgi are what I've been playing with.
