I haven't found anything that can do it well. Some tools like procoder are great for choosing the chapters you want from a DVD, but it uses the Quicktime encoder which is a PoS. X264 is the best encoder, but it's hard to find a good front end for it. You might want to take a look at Handbrake, but I wasn't very fond of it. I'm making do with Media Coder and selecting in and out points manually.

The trade off is ease of use vs. quality encoding.

We use Apple compressor with the true H.264 encoder. The best thing to do is pull your DVD content out using an up-converter to make it DV quality so you have 0% loss

I didn't think Apple supported the h.264 high profile at all. The quality difference between the main and high profiles is night and day.

For me right now, Id rip the dvd to mpeg2 then convert it from there. Are you using this for ipod, or for high quality streaming vids? I haven't found anything to batch convert to h.264 yet that matches the quality of Main Concept plugin for premiere. videocharge supposedly updated for full h.264 compatibility, but on a test yesterday it just sat there. Sorenson 5 I think is also h.264 compat but I haven't upgraded from 4.5 yet.
