Actually offer the product you promise and 99.5% will not request a refund. You're doing something wrong if you even notice that 0.5%.

I'm not sure - even with a strong "policy" in place and 100% fulfillment etc we still get more than .5% (1 in 200)

Anything higher than 1% means you're doing something wrong.

You're right, that's a lot of difference, though - what I am saying is even a very good service has 1 in 200 chargebacks, or .5%, so it matters. The shit they put you through if you reach the 1.4 threshold is pretty time consuming.

1.4 is a 40% increase in chargebacks over 1%, that would be cause for concern.

Like knowing you need to change the oil and driving after the oil idiot light goes on
