Where did you read that the doctor declared him unfit for flying? It's a "sick note" as far as I know, which could mean any of 100 different things.

he had no obvious injury and if he had the flu and fever I guess someone would have noticed

Planes change altitude as part of routine flight. Usually no one notices these changes. I usually don't; I am usually wrapped up in a movie doing everything I can to block out all of the noises from the plane.

Most likely they knew something was wrong in the final few minutes when the pilot was trying to hack down the cockpit door with axe.

You would have thought that he would have responded to the radio even if he ignored requests - almost all suicides leave a note to say why they are doing it and I would have thought that he would have done that over the radio. keeping totally silent is strange.

From reading the main pilots forum, it's clear that much of that industry just doesn't get it. Anything irrational goes against the grain of being a pilot, and thinking like a pilot. People try to discuss mental health issues, but they're shouted down as being full of shit.
